University of New Mexico Health System, Genesis Healthcare partner on clinical pathways to reduce hospital readmissions

The University of New Mexico Health System is partnering with Genesis HealthCare to create clinical programs to improve care access and reduce hospital readmissions among nursing facility residents. The clinical programs will take place in the Albuquerque area, where Genesis operates 10 facilities, and other parts of the state to address issues that arise during transitions of care between hospitals and long-term care facilities. Clinical pathways will be designed for orthopedics, pulmonary care, cardiac care, neurology, recovery and behavioral health for patients discharged from acute care hospitals and admitted to follow-up facilities for continued care. The partnership will emphasize:

  • Clinical coordination;
  • Physician oversight during care transitions; and
  • The national shift to value-based care

Genesis will provide beds for UNM Health System patients at its locations, along with a customized admissions process.
